Are you an enthusiastic self-starter looking for an impactful role creating positive onboarding experiences for children and young people living with significant disability?Be the Place You Want to WorkAttractive salary and salary packaging to $15,900 Fitness Passport | Diversity and Inclusion | Hybrid work environmentWesthaven is seeking an Intake and Onboarding Officer to ensure a responsive and proactive intake and onboarding experience for children and young people living with significant disability. About usWesthaven is and will continue to be the provider of choice for disability support services and accommodation across regional NSW.  Westhaven provides client-centric, high-quality programs and activities that provide whole-of-life support for people with disability, including offering houses that are homes, enabling choice and control and supporting clients to live life the way they choose.  About the roleWesthaven’s Intake and Onboarding Officer will ensure a responsive and proactive intake experience for prospective children and young people, their families and referring agencies to provide excellent customer service and support the continued growth of Westhaven’s Children Services.Key responsibilities:Liaise with funding bodies, external service providers, families and carers to ensure support and services are provided to vulnerable children and young peopleWork as part of a high-performing multidisciplinary teamCollate referral documentation and deliver to panelComplete responses for referrals Undertake placement risk assessments and client mix compatibility assessmentsComplete child / young person placement planning and reportingLiaise with internal stakeholders to deliver the best outcomes for children and young peopleCoordinating transition of the child / young person into placement Coordinating case management transfer, including ensuring appropriate exchange of documentation and information from previous case management agencyComplete funding applications in line with funding body requirementsCreate child / young person onboarding welcome packsAs a flexible and understanding individual, ideally experienced within the disability care sector, you will:Have the ability to organise a team to meet positive outcomes for children and young peopleBe skilled in assessing the needs of children and young people; including risk assessments, action plans and be able to mitigate any identified risksHave an understanding of the legislativements and regulations of the National Disability Insurance Scheme (NDIS), the Department of Communities and Justice (DCJ) and the Office of the Children’s Guardian (OCG)Be able to interpret Positive Behaviour Support PlansHave demonstrated knowledge of the principles and practices relevant to delivering quality services within Statutory Out of Home Care Have advanced problem solving capabilities, confidence and emotional intelligence that enable you to resolve complex situationsEssential requirements of the role include:A current NSW Driver’s licence The ability to obtain and maintain a paid Working with Children Check (WWCC)The ability to obtain and maintain a National Disability Insurance Scheme Workers Check (NDISWC)Appropriately vaccinated against Covid-19 and able to provide evidence About applyingPlease submit your CV and a cover letter addressing your abilities to undertake the key responsibilities of this role.
